A Lead Care Manager - Licensed Clinical Social Worker (LCSW) is responsible for overseeing the care management team and ensuring that patients receive comprehensive and coordinated care. This role involves developing care plans, coordinating services, and advocating for patients' needs.
Key Responsibilities:

Requirements
Education: Master's degree in Social Work (MSW) from an accredited program.
Licensure: Licensed Clinical Social Worker (LCSW) certification.
